The administrator is a provider of health services pursuant to Act No. 372/2011 Coll., on health services.

#II. Purposes of processing personal data

We process your personal data for the purpose of:
– provision of health services
– recording and reporting of medical services
– billing for unpaid care
– informing you and authorized persons about your health status
– patient booking and care organization
– accounting and tax records

#III. Legal basis for processing

– legal obligation pursuant to Act No. 372/2011 Coll., Act No. 48/1997 Coll., Act No. 563/1991 Coll., and others
– contract for the provision of healthcare

#IV. Recipients of personal data

The recipient may be:
– other health service providers
– public authorities
– persons authorized to view medical records according to the law
– contractual processors (e.g. accountants, IT support), always in accordance with GDPR
We do not transfer your data outside the EU.

#V. Data retention period

– according to Decree No. 98/2012 Coll. on medical documentation
– we retain other data for as long as you are our patient and for one year after the end of care

#VI. Your rights

You have the right to:
– to access your personal data
– to correct data
– to restrict processing in cases provided for by law
– for deletion (except for medical documentation)
– data portability (within legal limits)
– file a complaint with the Office of the Ombudsman (www.uoou.cz)

#VII. Right to object

If we process your data on the basis of a legitimate interest, you may object. In such a case, we will consider whether our reasons outweigh your rights.

#VIII. Obligation to provide personal data

The processing is a legal requirement. We cannot provide you with healthcare services without this data. The obligation also applies to legal representatives or guardians.

#Informed consent to treatment

I was clearly informed about the nature of the treatment, its benefits, risks and alternatives. I had the opportunity to ask additional questions.
I agree to the price of the treatment, unless it is covered by health insurance. I have read the current price list, which is published on the Smart Care clinic website or is part of the treatment plan agreed upon by me.
Treatment guarantee:
We provide a 2-year warranty on selected treatments, provided that you undergo a preventive examination at least once a year and dental hygiene twice a year, or as recommended by a dental hygienist.
Operating fee and cancellation rules:
The operating fee for a 1-hour appointment is CZK 2,000.
The appointment must be canceled at least two working days in advance (i.e. 48 working hours).
For each appointment you book, knowledgeable staff, doctors, assistants, surgeries, devices, and the entire capacity of the workplace are carefully prepared.
If you do not show up for your appointment and do not cancel it in time, you will be charged the applicable operating fee corresponding to the length of the reservation on your next visit.

#Consent to the transfer of information

I agree that information about my health condition may also be communicated to me by telephone or e-mail, if the situation allows and I will be properly identified with a personal password.
I agree that the information will also be passed on to the persons I have named who identify themselves with the same password.
I have truthfully filled in all the information about myself on the first page of the medical history questionnaire. All the attached informed consents are strictly related to the name and surname, date of birth, social security number and address, which are already part of the filled-in data on the "Medical History Questionnaire" page, which is an integral part of the medical documentation.
